Can you provide a list of forms that will be required for processing my reimbursement?

There are five forms TEA will collect from you:

  • An Application for Texas Identification Number that establishes a payee ID number, which is required for each state review panel member. See the detailed instructions in the state review panel handbook on how to fill out this form.

  • A Direct Deposit Authorization, which is not required but is strongly encouraged

  • A Travel Questionnaire to give TEA details about your method of travel, anticipated travel expenses, and special accommodation requests

  • A Meal Reimbursement Worksheet to track daily meal expenses

  • A copy of your detailed flight itinerary if you will be traveling to Austin by plane (The itinerary must include the traveler’s name, travel dates, times, airline, flight number, fare, and the last four numbers of the credit card used to pay for the tickets.)
